| 15 disciplined after death |

MESA, AZ - The warden of a state prison in Florence was among 14 Arizona Department of Corrections employees disciplined after the death of an inmate last year. The DOC said John Ontiveros' last day was Jan. 16 and he has applied for his retirement benefits. He was warden of the Eyman Complex and had worked for the DOC since September 1986. Read more.
