| Report: Errors contribute to rise in suicides |

MASSACHUSETTS - A rise in the number of prison suicides and suicide attempts is due in part to a failure to more closely monitor inmates at risk, according to a report in the Boston Globe. There have been 15 suicides by inmates in the Massachusetts prison system since early 2005, and hundreds of more attempted suicides or instances of inmates wounding themselves. Correction officials say the suicides are random and unrelated, but an investigation by the Boston Globe found that decisions and errors by corrections officials are contributing to the rise. Among the problems were botched background checks, missing medical or mental health records or skipped security rounds. Read more.
