| Prison death leads to firings |

ONTARIO, CANADA - Two managers have been fired and two more suspended without pay for their actions at the time a young prisoner died at Grand Valley Institution for Women in Kitchener, Ont. Public Safety Minister Stockwell Day's office says the firings and suspensions are the result of a disciplinary investigation conducted after 19-year-old Ashley Smith died last fall. Smith, from New Brunswick, was found unconscious in a segregation cell Oct. 19 and later pronounced dead in hospital. An autopsy determined she died of "self-initiated" asphyxiation. Day spokesman John Brent says the Correctional Service investigation found certain managers responded in ways that were "inconsistent with their management accountabilities and responsibilities."
