Department of Corrections officials had been warned that inmates might try to forge their way out prison months before two convicted murderers managed to walk out by doing just that.

In a memo distributed over the summer, officials were told a "perfect storm" was primed to hit.

An employee of the Orange County Clerk of Courts caught a case of an inmate using forged documents to go free two years ago but when the same inmate tried it again this summer, it worked, Channel 9's Kathi Belich learned.

The memo was sent by the Florida Department of Law Enforcement to the Florida Department of Corrections and prosecuting attorneys in July.

The memo outlines four cases of forged court documents used by state prison inmates and a jail inmate to try to go free or shorten sentences.

The first case goes back to 2009 and involved Pinellas County inmate Nydeed Nashaddai.

The brief also mentions Orange County attempted murder convict Jeff Forbes' attempt to shorten his life sentence.

Earlier this month, inmates Joseph Jenkins and Charles Walker were able to successfully go free using forged documents.

The brief directs the Department of corrections to tell court clerks about the forgery scam, but two and three months later, the Orange County Clerk's Office was duped and set the wheels in motion for two murderers to go free from their life sentences.
