Offender Death Under Investigation

The Connecticut Department of Correction is conducting an internal investigation into an incident where an offender who suffered a contusion to the back of his head after a fall was taken to a local emergency room, and later died.

At approximately 12:59 a.m. on Thursday, January 9, 2020 at the Osborn Correctional Institution, Department of Correction staff members responded to an offender lying on his back on the floor of a dormitory area. The offender, who was conscious, was taken to the onsite infirmary for treatment. While there his condition began to deteriorate.

Staff initiated lifesaving measures and activated 911.

The offender was taken via ambulance to a local emergency room for treatment and was pronounced dead at 2:50 a.m.

After a preliminary review of the incident, and as part of its efforts to exercise due diligence, the Department of Correction has placed two Correctional Nurses on Administrative Leave effective Friday, January 10, 2020.

The deceased is identified as 36-year-old Jose Peralta of Waterbury. He last entered the Department of Correction on February 19, 2019 convicted on charges of Assault in the 3rd Degree, and Burglary in the 3rd Degree.
